| Professional Directory Application |
| In order to be listed on the Xtreme Lashes™ "Find A
Professional" directory, you must: |

Successfully complete the Xtreme Lashes Training and Certification Workshop.

Have qualified for and received your Xtreme Lashes Certificate.

Performed a minimum of 10 full applications (in addition to the 10 applications
done for your certification requirements).

Submit ‘before’ and ‘after’ pictures of at least 5 different full applications.

Download and fill out the form below and submit it via US MAIL along with your
photos for review.

| Xtreme Lashes™ must reserve the right to remove your
listing at anytime for any reason. For example, if we get reports of
unsatisfied clients of yours, we may remove your name. We will usually attempt
to discuss the situation with you prior to removing your name. |
| Xtreme Lashes™ receives thousands of visitors to its
website every month, many of them seeking professional technicians to apply
lashes to them. When you are listed here, you will receive a large number of
inquiries for your services. Once you have completed our Workshop and have
completed at least 10 successful applications with satisfied clients, we
welcome you to send in the form above and request your listing. |
